This will be for 3 troopers and 2 handlers max.

NOTE: Upon arrival, you will need to sign a form stating the following:

As a condition of being allowed to perform a service or provide visitation to patients, families, or staff members of WakeMed, I agree and attest to the following:
1. I do not have a fever, respiratory illness (e.g., cold, cough), rash, diarrhea, or vomiting.
2. I meet the minimum age requirement of 12 years of age, or the then-current minimum age requirement imposed by WakeMed (visitation restriction minimum age may change during flu season).
3. I have not been exposed to mumps, measles, pertussis, chickenpox, meningitis, COVID, or influenza in the last three weeks.
4. I agree to have a WakeMed staff member supervise the entire visit from start to end.
5. I will not perform any clinical activities, enter isolations rooms, or perform any other patient care work, including physical contact with any patient, documentation in the medical record, or advisement of other care providers or patients.
6. I will protect the privacy and confidentiality of all patient and other confidential information during and after my visit. I understand that patient and employee information from any source and in any form (paper, verbal, electronic) is confidential, and that business and operational data in any form is confidential and may qualify as a trade
secret.
7. I will not take any pictures or videos of patients or patient care areas and will not post to social media about my experiences at WakeMed.
8. I will adhere to all other aspects of the WakeMed Confidentiality Agreement.
